|概要||INTRODUCTION: Predictive factors for regional lymph node (LN) metastasis of gastric cancer have sporadically been described. In this study, we tried to comprehensively examine histological risk factors through a discrimination of lymphatic and blood vessels by immunohisotochemistry.
AIMS & METHODS: Gastric tissues were obtained from 240 operative cases with gastric cancer including 120 cases having LN metastasis [LN(+) group] and 120 cases without LN metastasis [LN(-) group]. Each group was consisted of 40 cases with early stage, 40 cases with invasion into the muscularis propria, and 40 cases with invasion into the subserosal space. Specimens were immunostained with antibodies for LYVE-1, von Willebrand factor (vWf), claudin-3, claudin-4, beta-catenin, MMP-1 and MMP-2. Various histological factors were compared between the two groups.
RESULTS: Tumor size was significantly larger in LN(+) group than in LN(-) group. Lymphatic invasion was observed mainly in the stroma near the primary tumor, and the number of cases with lymphatic invasion increased with depth of cancer invasion. Lymphatic invasion was detected in 54 cases in LN(+) group, which was statistically more frequent than in LN(-) group. The three cell-adhesion proteins tended to be expressed in cancer cells of low-grade malignancy, and the two MMPs conversely tended to be expressed in those of high-grade malignancy. Of these proteins, only MMP-1 expression was statistically predictive for regional LN metastasis. However, the extent of five proteins expression in the invasive front was not significant on lymphatic invasion.
CONCLUSION: LYVE-1 immunohistochemistry with combination of vWf antibody is useful for accurately discriminating lymphatic vessels from blood vessels. In gastric cancer, the three factors, such as tumor size, lymphatic invasion and MMP-1 expression, were predictors for regional LN metastasis.
